Case Management
Care Coordination is a specialized form of non-medical case management provided by AM/AA; it is funded through ISDH for persons with HIV infection. It consists of goal-oriented activities, which locate, create, facilitate access to and monitor the full range of HIV-related health and human services.
A Case Manager will make contact at least monthly to monitor changing needs and the appropriateness of the client’s service plan and offer a case update every three months to keep the client file active and open. Frequency and type of contact are based upon the client’s level of need. A Care Manager acts as a liaison between the client and service providers, whether in house programs or referrals, and facilitates implementation of the client’s plan of care according to agreed upon shared responsibilities. The Case Manager provides supportive counseling and encouragement when appropriate services cannot be found or have yet to be implemented.

- Providing assistance in negotiating complex programs and processes.
- Promoting the early use of health care and resources.
- Applying for and navigating the complex systems of insurance- Enrollment Specialist
- Educate and support the early use of preventative health care and resources.
- Assisting in securing financial resources including health insurance, employer resources, Food Stamps, SSI, Social Security Disability, medication assistance, state programs, substance abuse and housing programs.
- Linkage to appropriate services and support.
- Emergency Financial Assistance
- Pantry Program
- Ryan's Meals for Life (part of Meals on Wheels of Central Indiana)
- Mental Health and Substance Abuse Services through Volunteers of America
