AM/AA care coordinators assist clients with a variety of referrals.

They help clients determine their eligibility for State & Federal programs, assist in the application process, refer clients to local providers when appropriate through extensive inter-agency networking and advocacy for clients. This networking includes working with agencies which provide the following services:

  • Disease Intervention Specialists for accessing and informing past and present partners
  • SSI, SSDI Social Security programs
  • Medicare/Medicaid program
  • Visiting nurse service/ Hospice care service
  • Food Stamp Program
  • Health Insurance Assistance Programs (HIAP)
  • Psychological referral and testing services
  • Support groups
  • Medical testing and services (i.e. physician/dental referral)
  • Substance Abuse Program
  • Pharmaceutical emergency meditation, trials and compassionate use programs
  • Housing Programs: Rental Assistance Programs (HOPWA) & HUD Housing
  • Emergency medical, rental, utility, transportation assistance
